Диагноз обычно не представляет затруднений. Таким образом, задачей клинического обследования является оценка тяжести болезни для определения необходимой степени вмешательства. За исключением пациентов с жизнеугрожающим обострением БА, симптомы слабо коррелируют со степенью тяжести обструкции, так что следует проводить измерение ОФВ<sub>1</sub> или ПСВ. Серийные повторные измерения дают ценную информацию об ответе на терапию и помогают избежать как ненужных назначений, так и преждевременного снижения доз препаратов. Всем пациентам следует назначить ингаляционный бета<sub>2</sub>-агонист через небулайзер или с помощью дозированного аэрозольного ингалятора со спейсером (каждые 20 - 30 мин в течение 1 ч). В дальнейшем частоту проведения ингаляций следует корректировать в зависимости от степени улучшения бронхиальной обструкции. При тяжелых обострениях может быть назначена непрерывная небулизация - постоянное назначение небулизированного бета<sub>2</sub>-агониста в течение 1 ч. Было показано, что добавление антихолинергического препарата к небулайзерному бета<sub>2</sub>-агонисту вызывает дополнительную бронходилатацию [202]. Для поддержания SaO<sub>2</sub> выше 90% должно проводиться дополнительное назначение кислорода с помощью назальных канюлей или маски. Лечение должно быть начато сразу, как только установлен диагноз обострения БА и оценена степень его тяжести. Проводить повторные измерения функции внешнего дыхания следует периодически, через определенные промежутки времени, например через 2 ч после начала терапии.

При среднетяжелых, тяжелых обострениях и обострениях с неполным ответом на начальную терапию бета<sub>2</sub>-агонистами должно быть начато лечение пероральными ГКС, и оно должно продолжаться в течение 7 - 10 дней (например, преднизолон 40 - 60 мг/сут). Даже при легком обострении пациентам, регулярно принимающим ГКС, должны быть назначены дополнительные дозы перорального ГКС. Добавление ИГКС при снижении дозы пероральных ГКС уменьшает число рецидивов [224] и некоторые исследования показали, что ИГКС эффективны как препараты для продолжения лечения после отмены пероральных ГКС [225]. Лечение дыхательной недостаточности см. в разделе 17.

ПЛАНОВОЕ ЛЕЧЕНИЕ БРОНХИАЛЬНОЙ АСТМЫ

Все руководства по ведению БА рекомендуют корректировать интенсивность терапии в зависимости от клинической степени тяжести БА<sup> </sup>[226, 227] (табл. 8-32). Пациентам с эпизодическими симптомами, быстро купируемыми бронходилататорами, с нормальной функцией внешнего дыхания в промежутках между эпизодами (легкая интермиттирующая астма) не требуется никакого лечения, кроме ингаляционного бета<sub>2</sub>-агониста по потребности. Если симптомы появляются больше 2 раз в неделю, ограничивают активность, приводят к ночным пробуждениям больше

2 раз в месяц (легкая персистирующая астма), рекомендуется применение препарата для длительного контроля заболевания. Наиболее эффективной терапией является регулярное применение ИГКС, но антилейкотриеновые препараты, кромолин и недокромил, а также теофиллин рассматриваются как приемлемые альтернативные варианты. Для наиболее тяжелых форм БА (среднетяжелой и тяжелой персистирующей астмы) рекомендуются либо высокие дозы ИГКС, или комбинация ИГКС и ДДБА [228]. Невыполнения назначений - частое обстоятельство, к которому надо относиться с вниманием [229]. Как и при любом хроническом заболевании, условием успешного лечения является установление партнерских отношений с пациентом. Это, в свою очередь, требует обучения пациента, объяснения ему природы заболевания, целей лечения и действий, которые необходимо предпринять при усилении симптомов (план действий). Просто предоставления необходимой информации недостаточно. Самоведение требует овладевания определенными навыками, особенно навыками самооценки своего состояния (например, измерение ПСВ и интерпретация результатов) и лечения (например, правильное применение ингаляторов, правильное дозирование пероральных ГКС).

Вторая возможная причина плохого контроля астмы отражает отягчающие эффекты нелеченых сопутствующих заболеваний [230]. Таким пациентам, возможно, следует провести компьютерную томографию пазух носа и мониторирование пищеводного pH даже при отсутствии типичных симптомов синусита или эзофагального рефлюкса, поскольку все увеличивающееся количество клинических наблюдений свидетельствует, что лечение этих заболеваний может улучшить контроль астмы [231, 232].

Исследования кривых зависимости реакции от дозы ИГКС делает третью возможность - предположение о недостаточности дозы ингаляционного ГКС - менее вероятной. Возможно, у 30% пациентов с БА, особенно с длительным анамнезом заболевания и не имеющих признаков активности воспаления (низкая эозинофилия мокроты, низкий уровень NO в выдыхаемом воздухе), отмечаются незначительные изменения ОФВ<sub>1</sub> или бронхиальной гиперреактивности после лечения низкими и средними дозами ИГКС; кроме того, назначение высоких доз повышает подавление функции надпочечников, не улучшая каких-либо показателей функции дыхательных путей [233]. В отношении этих конечных точек кривая зависимости реакции от дозы вначале крутая, а в дальнейшем плоская, поэтому наибольшее улучшение достигается при низких дозах [234]. Однако нельзя быть уверенным, что отсутствие улучшения показателей функции внешнего дыхания указывает на отсутствие других преимуществ лечения ИГКС - снижения риска обострений.

Растущая осведомленность о характере кривой зависимости реакции от дозы и обеспокоенность возможностью токсичности высоких доз ИГКС при длительных сроках лечения вызвали интерес к эффективности добавления других препаратов для контроля заболевания. Из всех изученных комбинаций наиболее эффективно сочетание ИГКС и ДДБА. В ходе многих клинических испытаний было показано, что у пациентов со среднетяжелой и тяжелой БА, недостаточно контролируемой ИГКС, добавление к терапии ДДБА является более эффективным, чем удвоение дозы ИГКС, по показателям улучшения симптомов и количества ночных пробуждений, функции внешнего дыхания и качества жизни [228]. Одно время бытовало мнение о возможности улучшения контроля при добавлении ДДБА просто за счет того, что их бронходилатирующее действие маскирует бронхиальную гиперреактивность и усугубляет воспаление дыхательных путей [235]. Было показано, что монотерапия ДДБА контролирует симптомы и повышает пиковую скорость выдоха так же, как и ИГКС, но не контролирует воспаление дыхательных путей и не предотвращает обострения [236]. Именно поэтому ДДБА не рекомендуются в качестве единственного препарата для контроля астмы. Однако несколько проспективных исследований показали, что комбинированная терапия ИГКС и ДДБА снижает частоту обострений БА [237]: добавление формотерола к низким или высоким дозам будесонида снижает частоту как легких, так и тяжелых обострений, причем наиболее низкий уровень обострений наблюдается при терапии комбинацией «высокие дозы будесонида/формотерол» [238].

Другие исследования показали, что добавление теофиллина к низким дозам ИГКС повышает терапевтическую эффективность лечения, но не является более эффективным, чем удвоение доз ИГКС. То же самое относится и к антилейкотриеновым препаратам. Однако была продемонстрирована большая эффективность добавления ДДБА к ИГКС по сравнению с добавлением зафирлукаста или монтелукаста [239].

Выделяется подгруппа пациентов с хронической тяжелой астмой, которая характеризуется высокой потребностью в медикаментах или в персистировании симптомов, рецидивировании обострений или в сохранении бронхиальной обструкции, несмотря на высокие дозы препаратов. Термин хроническая тяжелая астма включает несколько вариантов тяжелого течения БА, такие как рефрактерная астма, стероидозависимая астма, трудноконтролируемая астма (difficult-to-control), нестабильная (brittle) астма и бронхиальная астма с необратимой обструкцией. Эта подгруппа логично должна включать пациентов со стероидорезистентной астмой, но обычно они рассматриваются как особый и отдельный фенотип [240]. Пациенты с атопической тяжелой астмой и IgE-опосредованным ответом, в том числе и принимающие системные ГКС, представляют собой тот фенотип, для которого была показана эффективность применения анти-IgE-моноклональных антител (омализумаб) в отношении снижения доз ингаляционных или пероральных ГКС, необходимых для контроля заболевания [241].

Муковисцидоз (МВ), или кистозный фиброз поджелудочной железы, - моногенное заболевание, обусловленное системной дисфункцией экзокринных желез. Клинически оно проявляется прогрессирующей инфекцией и обструкцией дыхательных путей, экзокринной недостаточностью поджелудочной железы, нарушением процессов пищеварения и всасывания в кишечнике, холестатическими осложнениями, а также снижением репродуктивной функции у мужчин в связи с недоразвитием или отсутствием семявыносящих протоков.

Болезнь впервые была описана в 1938 г. Дороти Андерсен, патологоанатомом Нью-Йоркской детской больницы. Она опубликовала первое подробное описание кистозной дегенерации поджелудочной железы в сочетании с легочной патологией у детей, выделив его в самостоятельную нозологическую единицу под названием «кистозный фиброз» (англ. - cystic fibrosis) [1]. Другое наименование - «муковисцидоз» предложил С.Фарбер в 1944 г. (лат. mucus - слизь, viscus - вязкий), указывая на роль повышения вязкости экскрета [2]. Уже тогда была очевидна ведущая роль хронической легочной инфекции в патогенезе МВ. С началом эры антибиотиков (середина 1940-х годов) связаны первые успехи в лечении МВ, когда Paul di Sant’Agnese применил пенициллин. Тогда наиболее распространенным бактериальным патогеном был пенициллинчувствительный Staphylococcus aureus [3]. В эти же годы Андерсен и Ходжес высказали предположение об аутосомно-рецессивном типе наследования МВ [4]. Важным открытием 1950-х годов явилось обнаружение повышенного содержания хлоридов в поте больных МВ во время жары в Нью-Йорке летом 1953 г. (тепловой коллапс у грудничков) [5]. Потовый тест, разработанный Гиббсоном и Куком, стал «золотым стандартом» прижизненной диагностики МВ [6]. До этого времени считалось, что МВ приводит к смерти больного в 1-й декаде жизни; однако в начале 1960-х годов появились первые сообщения о больных, достигших подросткового и взрослого возраста. Тогда же были введены в практику программы активного непрерывного лечения МВ, которые продолжают совершенствоваться и сейчас [7 - 10]. В дальнейшем были созданы специализированные центры для лечения больных МВ, что привело к значительному увеличению выживаемости и росту числа взрослых больных [11 - 14]. В настоящее время предполагаемая выживаемость больных Великобритании, родившихся в 2000 г., приближается к 50 годам [15].

Муковисцидоз - не только наиболее частое моногенное заболевание, но и яркий пример достижений современной молекулярной биологии в изучении наследственных заболеваний [16]. В 1989 г. совместными усилиями канадских и американских ученых под руководством д-ра Лап Чи-Тсуи был открыт ген МВ. Новый ген получил название гена Трансмембранного Регуляторного Белка Муковисцидоза (МВТР) [17, 18]. Проблема доставки МВТР-гена в эпителиальные клетки с целью получения терапевтически значимой дозы белкового продукта продолжает оставаться центральной во всех проектах по генной терапии.
